Dates have been set with Dr. Sherry Shamp and Dr. Joseph Shamp to house four days of physical's here on campus at Sarasota Christian School.

FHSAA's requirements to participate:
Yearly physical for all participants (EL2 form)
All students must have had an ECG (Electrocardiogram) screening before participation (EL1 form) - Not required every year, but it is recommended.

All forms need to be uploaded to Athletic Clearance. The athletic department does not accept paper copies. Click here for more information about required forms and Athletic Clearance. 

Blazer Athletics has set four dates for physical evaluations, which you can add an electrocardiogram screening onto the end of:
June 3rd from 1-3pm
June 12th from 12-2pm
June 15th from 12-2pm
July 15th from 1-3pm

Signups for physicals will be through Signup Genius, which will ask if you wish to add on the electrocardiogram screening as well.

Cash is the preferred payment method and payable on the day of at the examination.

This year, physicals will take place in the Athletics Field House, with the conference room working as a waiting room and boys and girls locker rooms working as the examination rooms. Paperwork to fill out will be sent out to each family prior to the day of the examinations in order to make the process quicker for everyone.

The examinations take 15 minutes, so we have room for sixteen each day.

Note: We are waiting to hear back from the FHSAA about ECG Screenings done in the past as well as ECG's done by middle school students.
